Clarification on Demotions

When a user is demoted from any rank, they are not entitled to a warning. However, if the user makes a mistake that is easily repairable, then a warning may suffice in place of demotion.

Example 1:
A moderator swears on OldCP. A warning would not suffice here, as swearing is a known rule and a moderator should not have to be moderated.

Example 2:
A moderator does not meet the activity requirements for two months in a row, without making an inactivity notice. A warning would suffice here, as that moderator may be able to improve their activity immediately.

Example 3:
A Wise Moth is found to leak the content of private scrolls. A warning would not suffice here, as it is a known rule that Moths are supposed to keep the scrolls protected.

The general rule of thumb is this: Should this user know better, or was this a mistake that can be easily fixed?

I'll always be understanding. If someone is genuinely sorry and shows this to me, I will forgive them and give another chance. However, some mistakes should not be given second chances without obvious regret.

Well stated. Action has to be taken based on the infraction; like everything else, it's situational, and history has to be taken into account. There is no one size fits all.
