What about co-op Blast + Skirmish mode in Wrath Games? Spawn with nothing but iron items and every horde you defeat randomly gives you:
- a more powerful weapon (melee, bows, sabers or even guns)
- better armor
- temporary power-ups (lightning, firebreath, etc.)
- military vehicles (jets, helis and tanks with missiles and bombs)

If you complete all hordes or lose all your lives, you will respawn in Wrath Caverns with your normal stuff and a reward depending on the number of hordes completed.
Also, Health will be the same for all players, except for Strength to encourage people to train harder in order to make power-ups last longer.

How cool is that?
